Do I need to answer every question to submit an injury form?

You don’t need to answer every question to submit an incident form, but some questions are mandatory. When the 'NEXT' symbol appears, it means you have answered all the mandatory questions on the page and can now proceed. If it doesn’t appear, continue filling out the fields until it does. 

We recommend taking the time to provide as much information as possible. The more information, the better your injury record will be. 

If you don't have time to add all the detail, you can also choose to return to the injury form and add more detail later.
